THE DREAM - The end of times. It was the end of times, or the end of the world, or some nuclear war. Some people were not realising this.

THE REALITY The dreamer had suffered serious diarrhoea for twelve days. He wanted to go to the doctors but was nervous going there whilst he had such bowel problems. It was starting to dawn on the dreamer that this could be the end game. That if he had serious diarrhoea for life then life would be unbearable.

THE INTERPRETATION If we link this dream to recent events in the dreamers life then having serious diarrhoea is bound to be the one abounding issue. So this metaphor was quite simple to see in the context of this.
